Wellness is a consistent state of good health — both for the mind and body. We help our patients achieve wellness through lifestyle changes, regular monitoring and mental health care.
We guide patients to maintain a healthy diet and good exercise habits, two keys to wellness. Patients can enroll in our weight management program, which facilitates a healthier weight through a personalized weight tracking system and monthly progress checks.
Good mental health is essential for wellness. We provide therapy to patients and their families to address stress, anxiety, depression, caregiver distress, grief and other symptoms caused by chronic or serious illnesses. Our therapy services are reimbursed by Medicare and most major insurance providers.
We also provide memory screening services to detect cognitive impairments, such memory loss and decreased mental processing speed. We give patients an initial screening and conduct follow-up testing to monitor their cognitive function. If we detect cognitive impairments, we refer patients to an internal medicine physician or a neurologist.
We offer Medicare Annual Wellness Visits to patients who have had Medicare Part B for over 12 months. In these yearly visits, we conduct a basic checkup on patients’ overall health, provide medical advice and develop a personalized plan to prevent illness. Please note that an annual wellness visit is not a head-to-toe physical. Wellness visits are similar to — but a separate service from — the one-time “Welcome to Medicare” preventative visit.
