Our mission is to provide young adults easy access to healthcare as they are transitioning from pediatric care. We aim to be the point of contact for health related questions so young adults can make a successful transition to the adult health care system and ensure they receive preventive services that help them remain in good health.

Meet Kim Warnock, NP-C

Kimberly (Kim) is a Family Nurse Practitioner who brings nearly a decade of experience providing exceptional care for both acute and chronic illnesses. Much of Kim’s experience has been in the Emergency Department where she treated a wide range of health complications. Through this experience, she has seen the true value of comprehensive preventative medicine, young-adult care, diabetes care, and management of chronic illness in patients which has become her passion.
